How to Apply for the NDIS

1. Click this to download the Read the NDIS Fact Sheet for further information 

2. Check your eligibility - refer to NDIS Website for further information - https://www.ndis.gov.au/applying-access-ndis/am-i-eligible

3. Complete the NDIS Access Request Form, along with supporting documentation and email to enquiries@ndis.gov.au. 

4. The NDIS will make a decision within 21 days.
